Vitamin D, The Sunshine Vitamin






It has been estimated that up to 30% of Australians are Vitamin D deficient and vitamin D supplementation has become a hot topic amongst health care professionals. Although, some current opinions support increases in supplementation dosage levels, there is also substantial evidence to suggest more caution should be used in relation to high dosage or long term vitamin D supplementation use.


Vitamin D is not like other vitamins and it is questionable if it should have been classified as a vitamin. Vitamin D is actually a pro-hormone; meaning it promotes or inhibits the effect of hormones. The major hormones that vitamin D influences includes; oestrogen, progesterone, testosterone, insulin, thyroid, parathyroid and adrenal hormones. As such, the role of vitamin D within the body is complex

Vitamin D is stored in the blood for a few weeks and in fat cells for a few months. The liver and kidneys help to convert vitamin D into it's more active form.

Vitamin D deficiency has been linked to the following conditions:

Bone Health:

Osteoporosis: Vitamin D facilitates bone mineralization of calcium

Rickets: a deficiency of vitamin D during pregnancy, breast feeding and early childhood, can result in this condition of decreased bone mineralization.

Cancer:

The latest research indicates a link between vitamin D deficiency and breast, prostate and colorectal cancer

Immune Function:

Type1 diabetes, asthma, psoriasis, multiple sclerosis, rheumatoid arthritis, systemic lupus and inflammatory bowel disease, are all auto-immune conditions that have been associated with a vitamin D deficiency.

Type 2 Diabetes:

Vitamin D is involved in pancreatic function and blood glucose metabolism.

Mental Health:

Fatigue, depression and seasonal affective disorder have been associated with low vitamin D levels. Perhaps, this is one of the reasons why our mood lifts, when we get some spring sunshine after the winter months

Fertility And Reproduction:

 Male Fertility: receptors for vitamin D are present in sperm and are linked to sperm survival and their ability to bind to and fertilize the egg.

Female Fertility: vitamin D is essential for proper implantation of the embryo in the womb.

Pregnancy: vitamin D deficiency in pregnancy is associated with an increased risk of developing gestational diabetes and pre-eclampsia. The child is predisposed to low birth weight and rickets


How To Optimize Your Vitamin D Stores

Diet

We rely on dietary sources for only 10% of our vitamin D requirements. Rich sources of vitamin D are mackerel, herring, salmon, sardines, egg yolk and butter.

Sunshine

90% of our vitamin D is made from the cholesterol in our skin in response to UV-B penetration from the sun. So, the most basic way for us to get our vitamin D is through exposure to sunlight. Clothes, glass and sunscreen will block UV-B penetration and so inhibit vitamin D synthesis

UV-A rays destroy vitamin D and are linked with skin cancer risk. Unlike UV-B rays, UV-A rays are able to partially penetrate glass. So when we are outdoors we get a balance of both UV-A and UV-B rays. Whereas, indoors we get some exposure through glass windows to UV-A only.

Safe Sun Exposure

The success of the slip. slop, slap campaign has resulted in us becoming some what sun phobic. Hence, we are now seeing the re-emergence of rickets in Melbourne school children.

We have been taught that the safest time to spend in the sun is before 10.00am and after 3.00pm. More recent research [2008] dispels this by showing that UV-B ray exposure is highest between 10.00am - 2.00pm. Therefore, daily "sunscreen free" sun exposure between 10.00am- 2.00pm will provide you with the maximum vitamin D yield

The amount of time required will depend on your skin type. Those with darker pigmented skin or a tan will need longer in the sun than pale skinned people. Basically, sun exposure needs to be long enough to cause a slight pinking but not reddening of the skin. This can range between 10-20 minutes. The amount of skin exposed needed is face, arms and upper body or face, arms and legs.

Overdosing on vitamin D due to sun exposure is not likely. This is because there is a feedback loop and the body will shut down vitamin D production, when your vitamin D levels are optimal

Sunscreen

Please note, I am not advocating long periods of sun exposure without protection. As we approach the summer holidays, we are likely to be spending more time outdoors and slapping on the sunscreen lotion.

Unfortunately. most sunscreen lotions are a cocktail of synthetic chemicals; some of which will be absorbed into your blood stream and can impact on your health. Firstly, the chemicals used tend to produce a lot of cell damaging free radicals. Secondly, many of the chemicals are oestrogenic and can disrupt your hormone balance

A good natural sunscreen I have been able to source is "UV Natural" and I have found it to be equally effective in terms of sunburn prevention as the synthetic sunscreens.To get more information on this product you can visit www.uvnatural.com However, it is a little on the greasy side but I have found the sports formula to be less greasy & I for one would much rather be putting this on my skin than a whole load of synthetic chemicals.


Factors That May Reduce Vitamin D Synthesis
  1.  Dry skin: excess soap and cleaning chemicals can change the barrier function of the skin and negatively impact on vitamin D conversion from cholesterol in the skin.
  2. Use of cholesterol reducing statin drugs.
  3. Imbalances or deficiencies of magnesium, zinc, boron or vitamin A and K. These all have a synergistic relationship with vitamin D.
  4. Bowel conditions such as crohn's disease, coeliac disease and irritable bowel due to reduced absorption abilities

NB

Please do not self-prescribe vitamin D supplements as this may lead to imbalances that impact on your health. However, the amount of vitamin D in a multi-vitamin is fine. Requirements are individual and the need for additional vitamin D supplementation is best assessed through a combination of your blood vitamin D levels and a mineral hair analysis, as well as taking into consideration your metabolic type and current health profile

Lao Tzu is considered to be he father of Taoism in China & is credited with being the author of the ancient Taoist text the "Tao Te Ching." Although his historical existence & who actually authorshiped the "Tao Te Ching" is strongly debated, this does not distract from the wisdom of the teachings in the "Tao Te Ching."

The wise women waters her own garden first........this is something we definitely need to heed for our own ultimate wellbeing. Particularly, as most of us western women have been taught to give fully of ourselves without taking the time to rest & rejuvenate. Our society has forgotten the age old wisdom that recognizes that in order to fully give, we first need to receive & take care of ourselves. 

Ask yourself if you have developed the pattern of "over-giving" & consequently have neglected your own needs? If you have, then you will recognize the following thought patterns: 
  • "I must be a perfect image of having it all together & able to support others"
  • "Being busy means I am doing a good job & getting things done"
  • 'How worthy & good I feel about myself depends on how much I do for others or add value to the world"
  • "Taking time for myself is selfish & my friends, kids, partner take up all my time anyway"

Failure to take time to nourish & replenish ourselves leaves us exhausted & depleted & quite frankly most women I see in my clinical practice who have reached 40 plus are exhausted. If this pattern is not corrected before the menopause transition it can trigger a lot of imbalances at this challenging time of change in a women's life.

Extreme fatigue such as when you reach the point of being so tired you just want to cry; is a sure sign that you need to do absolutely nothing except totally rest. However, fatigue that persists should be investigated to determine if there are any underlying causes such as anaemia or thyroid imbalances.

When you are feeling fatigued the tendency can be to reach out for stimulants such as coffee, chocolate & sugary drinks or food. Whilst these may temporarily give you a boost of energy, the problem is that long- term these stimulants deplete your vitality & inhibit restful & restorative sleep. This is because they they put your body in a constant state of fear, fight or flight mode.

Improving Egg Quality & Ovarian Resposiveness



Although, in this blog I am focusing on ways to support egg quality & ovary responsive to hormone signals; I would like to emphasize that the health of the male partner & therefore his sperm quality is equally important. As I mentioned in my previous blog post titled "Am I Too Old To Have A Baby,"in men and women who are 40 & over, oxidative stress is a major consideration & antioxidants a must.  For instance when the sperm penetrates the egg, the egg will use it's own resources to to repair any damage & oxidative stress to the sperm first & then use what is left to repair any oxidative damage to itself. So the healthier the sperm the less demand this places on the eggs own resources.

Medical Tests That Assess Ovarian Reserve & Responsiveness

Follicle Stimulating Hormone [FSH]

This blood test should be performed on day 2 or 3 of the menstrual cycle.
It is not a particularly sensitive marker as it will only be elevated in cases where their is severe depletion.

Anti-Mullerian Hormone [AMH]

This has become knowm as the "egg timer test" & has only been available for a few years.
AMH is a protein hormone secreted by the immature cells of the ovarian follicles in response to stimulation by FSH. This blood test should be performed on day 3 of the menstrual cycle & is thought to provide an estimation of the number of early growing follicles. Low ovary reserve maybe indicated with an AMH less than 10pmol/L.

However, egg quality & ovarian responsive is more important than the number of eggs in reserve. If the ovaries response to hormone signals improves then the AMH levels will also be seen to increase.

Studies on women undergoing IVF treatment have shown that regardless of the women's age, AMH was predictive of the number of eggs retrieved but was not predictive of pregnancy rates & outcomes.

Antrial Follicle Count

This investigations the most accurate assessment of ovarian reserve.
It involves a trans vaginal ultrasound in which the mature antral follicles are counted.


Nutrients That Improve Poor Egg Quality And Ovarian Responsiveness

The nutrients I am about to mention will need to be taken for 4 months to have the effect of improving egg quality as this is the time that it takes for the egg to ripen & mature. Also, please note that there are many other nutrients & factors that effect fertility & what I am about to share here relates specifically to supporting egg quality & ovarian responsiveness or for women who are 40 & over.

Coenzyme Q10
This directly improves the energy production within the cells & so improves egg quality & longevity
For fertility you will need to take 200mg twice daily.

Vitamin C
Vitamin C is a powerful antioxidant & immune support. The ovaries are known to be a site of vitamin C accumulation & vitamin C aids the growth & repair of ovary follicles.
The dose is between 2-4 grams daily, ie 1 or 2 grams taken twice daily.

Vitamin E
Vitamin is another powerful antioxidant & so protects the egg from damage & also promotes hormone balance. It improves the integrity of the mucus membrane linings of the fallopian tubes & uterus lining [endometrium].
The dose is 500 i/u twice daily

Vitamin D
On the outside of the female egg there are vitamin D receptor sites & these determine what signals will be released to aid the sperm in finding the egg. Approximately 20 sperm find their way to the egg, but only one gets to penetrate the egg & vitamin D is probably the most important component in that process.

Vitamin D also aids implantation of the embryo & can normalize ovarian response to FSH.

You will need to get your vitamin D levels checked & supplement if needed, for conception & fertility you want to aim for your Vitamin D stores to be 120 mmols in winter & 150 mmols in summer.

Arginine
This is an amino acid [amino acids are the building blocks of proteins] which has been shown to be effective in improving egg quality & it is best used in poor responders to IVF treatment. It can improve the number of mature follicles, improve ovarian response, aid implantation & improve pregnancy rates.

As a supplement it is in a powder form & will need to be prescribed by a naturopath or nutritionist.

Another amino acid called Inositol has also been shown to have a similar effect as arginine, which emphasis the importance of quality protein in the diet for conception & fertility & this is why I do not recommend following a vegan or strict vegetarian diet for my fertility clients & I encourage them to at least include some fish in their diet.

Understanding Your Menstrual Cycle


Your menstrual cycle is the result of the cyclic changes that occur in your body due to the fluctuations in the levels of various hormones. A normal cycle is generally around 27-32 days long and can vary from woman to woman, or cycle to cycle. The length of the cycle is measured from the first day of bleeding (spotting doesn’t count), which is called day 1, to the first day of bleeding of the next menstrual period.

Your cycle is composed of the following 4 phases:
Menses: This is your period (from the first day of bleeding] and a normal period would last 3-5 days.In a healthy cycle there should be no pain, flooding or clotting & the blood should be bright & start & stop cleanly [no spotting].

Follicular phase: The follicular phase, or the pre-ovulatory phase, extends from the beginning of the cycle to ovulation. During this phase, the ovarian follicles (which eventually give rise to the egg which is expelled at ovulation) develop and mature, and your body prepares for ovulation. Between 10-20 follicles begin to develop, but usually only one will become dominant, ripen & release the egg [ovum] at ovulation.

While the follicles are developing, they produce & release increasing levels of oestrogen which stimulates the growth of the lining of the uterus, known as the endometrium. Increasing oestrogen levels also support the production of increasing amounts of ‘fertile mucous’ & cause the cervix to soften & open to allow in the sperm.

The follicular phase should be no less than 13 days or greater than 16 days & for optimal fertility ideally between 13-15 days.

Ovulation:  AS oestrogen levels continue to rise this triggers the release of luteinising hormone [LH] & follicle stimulating hormone[FSH], triggering the release of the egg [ovulation]. Ideally for optimal fertility ovulation should occur between day 13-14, day 15-16 is suboptimal. If ovulation occurs from day 17 onwards & the egg is fertilized there is a high risk of miscarriage occurring.
 

Luteal phase: This is the post-ovulatory phase, the time from ovulation until the end of the cycle, and is generally 14 days long. The length of this phase is fairly constant for most women & variations are more likely to occur in the follicular phase. The luteal phase should be no less than 13 days with no spotting, pain or PMS symptoms. Less than 10 days would be an infertile cycle.

In the post-ovulatory/luteal phase of the cycle, the corpus luteum (the remains of the ovarian follicle) starts to secrete increasing amounts of progesterone, which causes:

  1. The lining of the uterus to continue thickening & ensure it is fully supplied with blood and nutrients so that it is ready for the egg

If pregnancy doesn’t occur, the corpus luteum shrivels up and stops producing hormones, the uterine lining breaks down and, with blood, leaves the uterus during menstruation.
 The Fertility Window

Studies show that there is essentially a 6-day window of fertility, ending with the day of ovulation. The most fertile day is generally a day or two before ovulation, with the chance of conception highest in the presence of the most fertile type of cervical mucus. According to the World Health Organization the probability of pregnancy is greatest (66.7%) on the day of peak mucus but decreases to 8.9% three days after the peak mucus day.Although the egg is viable for (at most) 24 hours after ovulation, sperm can live in the woman’s reproductive tract for approximately 3 days, which means intercourse 3 or more days before, and up to the day of ovulation, could result in pregnancy. Studies suggest that conception on the day after ovulation rarely occurs.
